If you are a baptized Christian, that's all you need in order to receive Holy Communion.
The worship bulletin at St. Philip's makes an invitation to come to the altar rail to receive, and there are even specific instructions as to how to do it. Standing or kneeling, receive the wafer of bread and/or the wine; you may sip from the common chalice, or have the Eucharistic minister intinct (dip) the wafer into the wine for you, or you may have the chalice of wine pass you by altogether if you cross your hands over your chest.
